Don’t Forget: Using the Right Forms Keeps Our Groups Strong

Our LWML CNH has a set of forms for you to use to let our LWML District leadership know about your group and keep us all connected. These forms keep communications clear, help groups run smoothly, and save time by making sure everyone uses the same forms.

All of our forms can be found on our website’s Resources page. You’ll find these forms in the Forms section of this page.

If you are a new group or a new individual member, you should submit one of the following forms:

• New Society/Group Application
• Individual Membership Form

If an existing group or an existing individual member has any updates, they should submit one of the following forms annually with any updates:

• Society/Group Record Form
• Individual Membership Form

Any group can request that a district representative attend their event to give LWML updates and encouragement to your ladies by submitting the following form:

• Request a District Representative at your Zone Rally or Other Zone Event

We have specific forms for your group to make remittances to our LWML CNH district. Funds for Mites are typically collected in mite boxes or from special offerings. Funds for Scholarships might come from special offerings. Your group can receive a group rate to receive the LWML Quarterly for your ladies. Pro-rate funds are collected based upon the number of members you have in your group and are used to pay a mileage stipend to your convention delegate for the District convention.
